Background In India, 28.6% of population use tobacco and around 5% suffer with alcohol use disorder. COVID-19 pandemic has significantly affected the Indian population. Although preliminary evidence suggests alcohol and tobacco use adversely affects the course of COVID-19 illness yet there is less data to establish such association. The goal of our study is to systematically assess the prevalence and effects of alcohol and tobacco use among COVID positive patients. The primary objective is to estimate the current and lifetime alcohol and tobacco use among COVID-19 positive patients in the Deoghar district, Jharkhand state, India. Secondary objective is to assess and compare the severity of COVID-19 illness and the complications among the alcohol and tobacco users. A cross sectional study design using a telephone-based interview with the COVID-19 diagnosed patients or the family members will be conducted. A semi-structured interview schedule will be used for the assessment which includes socio-demographic details, substance use details, psychological assessment, COVID-19 outcomes. HIPPA compliant software will be used for telephonic interviews and data collection, storage.
Objectives Our goal is to systematically assess the prevalence and effects of alcohol and tobacco use among COVID positive patients through this study. Primary objective: To estimate the current and lifetime alcohol and tobacco use among COVID-19 positive patients in the Deoghar district, Jharkhand state, India. Secondary objective: • To assess the severity of COVID-19 illness and the complications among the alcohol and tobacco users and non-users. • To compare the severity of COVID-19 illness and complications between alcohol and tobacco users, and non-users.
Study Methods Key research design: Cross sectional survey Research setting: All India Institute of Medical Sciences (AIIMS), Deoghar, Deoghar District, India. Study Population: Patients from Deoghar district tested positive for COVID-19 (both RT-PCR/Rapid antigen). The information will be collected either from patient or key informant (example-family member). Sampling design: Telephone-based survey by counsellors on a sample population (selected using simple random sampling) by using a semi-structured questionnaire. If the patient is unavailable, family member will be interviewed. Sample size: For primary objective- The prevalence of tobacco use disorders as per Global Adult Tobacco Survey is 28.6%(GATS2 (Global Adult Tobacco Survey) Fact Sheet, India, 2016-17, n.d.). The prevalence of alcohol use as per recent national drug use survey, 2019 is 14.6% (Ambekar, 2019). Hence, sample size is calculated based on the tobacco prevalence. Sample size with 95% C.I and 5% standard error on a population of is calculated as 306. Considering 20% non-response rate, the sample size estimated to be 366. For secondary objective- The sample size calculated is 2800 (https://www.openepi.com/SampleSize/SSCohort.htm) Inclusion criteria: • All the patients tested positive for COVID-19 • The positivity status recorded in the national portal • Patients residing in the district of Deoghar • Patients or family members providing the consent for the study • Patients with age above 12 years Exclusion criteria: • Patients who have active infection, acutely ill (currently tested positive for COVID-19).
Expected outcomes and use of results • The study will help to estimate the prevalence of alcohol and tobacco use among the COVID-19 patients • To compare COVID-19 illness severity and complications among alcohol and tobacco users compared to non-users • Association between severity of alcohol use and tobacco use with severity of COVID-19 illness and its complications.
