Background Over the past decade, Indonesia has been dedicated to enhancing healthcare quality and expanding access to healthcare for all its citizens through universal health insurance. The government has set ambitious goals, such as eliminating poverty-related diseases like TB and HIV by 2030, while also accelerating leprosy elimination strategies over the next five years.
Objectives In response to Indonesia's efforts to enhance healthcare quality and expand access, this research project is dedicated to addressing the critical gap in healthcare access for marginalized populations, particularly individuals living on the streets. The objective is twofold: first, to establish health preventive and early detection of tuberculosis, HIV, and leprosy among marginalized populations through the modification of the Posyandu Jalanan program in Yogyakarta within 12 months; and second, to develop comprehensive guidelines for community-based health services focused on marginalized populations within 7 months, while simultaneously promoting best practices of the Posyandu Jalanan strategy to increase public awareness and gain stakeholder support.
Study Methods To achieve these objectives, the research will be conducted in Badran and Bong Suwung, Yogyakarta, with participants including Harapan Fian staff, volunteers, collaborators, and beneficiaries. The recruitment process will involve identifying participants through SIHI Indonesia's early assessment and receiving support from Harapan Fian. The research will span 12 months, commencing after obtaining ethical approval. Inclusion criteria encompass individuals associated with Harapan Fian, including beneficiaries facing street living challenges, such as scavengers, beggars, street performers, vendors, hawkers, transgenders, sex workers, and pedicab drivers aged 18-60 years, who can communicate verbally. Exclusion criteria apply to those not linked to Harapan Fian or Posyandu Jalanan and individuals unable to communicate verbally. The study design is cross-sectional and employs a mixed-methods approach, simultaneously collecting quantitative and qualitative data at a single time point.
Expected outcomes and use of results The project's primary aim is to strengthen the Posyandu Jalanan program's capacity to address infectious diseases, ultimately improving healthcare access for marginalized populations and enhancing community resilience.
